Online Registration Instructions
The procedure for submitting an Athlete Agent registration online is as follows:
- Download the Athlete Agent Registration Form
- Complete the fillable PDF form and save the completed copy
- Upload the completed Form and the requested supporting documentation in the fields provided below. (See page 9 of the registration form for requested supporting documentation.)
- Pay the required registration fee(s) by check to the address listed on the registration form. Checks should be made payable to the “Louisiana Department of Justice.”
